The Augmented Role of DIF on Diagnosis of Glomerular Disease in a Study in Northern Part of Bangladesh

Abstract


Introduction: Direct immunofluorescence (DIF) study is an important investigation tool for the accurate diagnosis of glomerulonephritis (GN). This study was aimed to evaluate the pattern of glomerular diseases in the northern part of Bangladesh and to see the advanced role of DIF over light microscopic (LM) findings for accurate diagnosis of various glomerular diseases. Methods: This cross-sectional analytical study was carried out in the department of nephrology of Rangpur medical college hospital, tertiary care teaching hospital in the northern part of Bangladesh from January 2103 to December 2017. A total of 773 patients were included in this study. All biopsies were examined by both LM and DIF technique. Clinical information was recorded in a predesigned data sheet before taking the biopsy. Results: Among the 773 patients, males were 51.5% and 48.5% were females. The commonest presentation was nephrotic syndrome (53.7 %) followed by nephritic illness (11.8%). FSGS was the commonest GN in this study (21.4%) followed by membranous glomerulopathy (18.6%) and minimal change disease (MCD) (12.3%). Out of 773, DIF was done in 741 patients. Among them, 671(86%) patients show positive immunofluorescence findings, while the rest 102(14%) shows no immune deposits. IgM is the predominant immune deposits. DIF is helpful in confirming 05 cases of FSGS, 04 cases of MCD and 02 cases of membranous GN after light microscopy. Conclusion: Direct immunofluorescence forms an important diagnostic tool for the accurate diagnosis in various types of GN along with LM study.
