Nt-Probnp and Nitrogen Species Measurement As Useful Biomarkers for Detection of Asymptomatic Heart Failure in Patients with Type 2 Diabetes

Abstract

Insidious heart failure is associated or complicated hypertension, coronary artery disease, diabetes, chronic lung disease, atrial fibrillation, renal failure, depression, and anemia. High plasma NT-proBNP level found to be a predictive value in detecting latent left ventricular diastolic dysfunction. The aim of the study is to detect asymptomatic heart failure in outpatient type 2 diabetic patients using plasma NT-proBNP as a diagnostic biomarker of heart failure and nitrogen species as biomarker of endothelial dysfunction. A total number of fifty diabetic patients of both genders and twenty five healthy subjects were admitted in this study. Fasting venous blood samples were obtained from participants and the sera were separated for determination of NT-proBNP, glucose, glycosylated hemoglobin (HbA_1c %) and nitrogen species (nitric oxide and peroxynitrite). Fifteen patients (30%) had serum NT-proBNP that exceeded the upper normal limit (100 pg/ml). Serum NT-proBNP which exceeded the level 900 pg/ml was detected in seven (14%) patients while no one of healthy subjects had serum NT-proBNP exceeded the level 600 pg/ml. Significant high serum nitric oxide and peroxynitrite levels in diabetic patients were observed and compared with healthy subjects. Patients with serum NT-proBNP >100 pg/ml had significant low serum nitric oxide compared with those having serum NT-proBNP less than 100 pg/ml. We conclude that cardiovascular events in term of asymptomatic heart failure and endothelial dysfunction detected in high percent in type 2 diabetes by measuring serum NT-proBNP and nitrogen species.
