Abstract

Background: Antenatal care is one of the most effective health interventions for preventing complication of pregnancy and childbirth. The components of basic focused antenatal care are assessment of condition of mother and pregnancy, care provision and health promotion. Poor maternal health not only affects women’s survival but has serious public implications for the survival of their children as well. However, there is no enough study that tells whether those pregnant women who visit antenatal clinic get adequate basic components of focused antenatal care or not. Objectives: To assess the magnitude of pregnant women receiving basic components of focused antenatal care and its associated factors in public health institutions. Methods: Retrospective chart review cross sectional study was conducted from April 2015 to May 2015 among 252 cards of pregnant women that took antenatal care services in West Shoa zone, Oromia Region, Ethiopia. Stratified sampling technique was employed by stratifying all public health institutions; two hospitals and 23 health centres were selected by Simple Random Sampling technique. The total sample size of the study was distributed over each of the institute proportional to their size. And the required number of study subjects was selected randomly from each health facility for the Quantitative study and for Qualitative part purposively seven health care providers were selected. Data were entered and cleaned using Epi-info version 3.5 and was analysed using SPSS window version 20 software. Both bivariate and multivariate analysis with 95 % CI was applied to avoid any confounding variables. Finally, the results were presented in texts, tables and graphs. Results: Out of 247 pregnant mothers who visited health institutions for antenatal purpose only 13 % of them received all basic components and focused antenatal care. Urban resident mothers were 3 times more likely receiving all basic components of Focused antenatal care than mothers residing in rural areas [AOR,3(1.15-7.79)].Those mothers who visit Hospital for Antenatal Care purpose were 7.24 times more likely receive all basic components of Focused antenatal care than mothers visiting health centres [AOR, 7.24 (2.91–17.95)]. Staff work load, lack of laboratory examination, materials/supplies, motivation and incentives were variables that had influenced the women's basic components of Focused antenatal care services. Conclusion and Recommendations: Majority of pregnant mothers visiting health institutions for antenatal care purpose were not given all basic components of focused antenatal care. Staff work load, lack of laboratory examination, materials/supplies, motivation and incentives were the variables that had influenced the women's basic components of Focused antenatal care services. Thus, regular supportive supervision, recruited skilled professionals and motivate staffs and create conducive environment by availing the necessarily supplies and materials is mandatory.
