A Case of Chronic Pancreatitis with Stones may Present with Obstructive Jaundice and Diabetes: A Case Report

Abstract

 

Obstructive jaundice in chronic pancreatitis is generally caused by stenosis of the bile duct in the pancreas. Obstructive jaundice caused by the impaction of a pancreatic stone in the papilla is very uncommon. We report a case of obstructive jaundice caused by the impaction of pancreatic stones in the papilla. A 43 year-old male with chronic pancreatitis was admitted because of epigastric pain with diabetes mellitus. Ultrasonography revealed a strong echo with acoustic shadows (stones) with complex heterogonous echogenic mass in the region of the pancreas. Removal of stones and a pancreatico-jejunostomy was done. His jaundice was corrected and became well controlled blood sugar level without any drug. Impaction of a pancreatic stone in the papilla is a markedly rare cause of obstructive jaundice in chronic pancreatitis. A correct decision taken pre-operatively may be needed for perfect management in our setting of limited investigation facilities.
