Even though every malunited hand fracture requires no intervention, malunion deformities including rotational malalignment and angulation can cause weakness of grasp and pinch. In addition, diabetes mellitus, a general factor of an infection, is an important illness which should be taken into account before undertaking reconstruction. Therefore, malunited fracture of the first metacarpal concomitant with diabetes may pose as a challenging problem, which includes various considerations such as indications, fixation technique and timing of surgery. Of the five fingers, thumb stability is essential for most hand functions. In the present study, we presented a case with malunited fracture of the first metacarpal base which was accompanied adduction dysfunction of the thumb, concomitant with diabetes, who was treated successfully by miniplating and perioperative medical therapy.
