Microsurgical Neurolysis, Dorsal Transposition and Nerve Wrap of Morton’s Interdigital Neuroma: A Novel Technique

Leon Alexander

Abstract

The treatment of Morton’s neuroma is varied with multiple options, both surgical and non-surgical. However, surgical treatment is indicated for persistent, severe, and recurrent forefoot pain whereby conservative measures have failed. The choice of surgical technique is controversial and depends on the patient presentation and surgeon preference. The key mantras in the successful treatment of this challenging condition are thorough clinical evaluation, proper patient selection, preoperative counselling, and meticulous surgical technique. This report highlights the microsurgical neurolysis of the third intermetatarsal nerve, dorsal transposition and nerve wrapping with good functional outcome after a two year follow up. This is perhaps the first report of such a technique being used in the treatment of this painful forefoot condition. This technique offers the advantage of preservation of nerve function and alleviation of pain with minimal morbidity and better functional outcome.
