Introduction: Appendicular tie/knot syndrome is a very rare presentation of acute appendicitis causing intestinal obstruction. The literature has described intestinal knot syndromes such as ileosigmoid knots and ileal-ileal knots. We present a case of an appendico-ileal knot, an uncommon type of closed-loop blockage, caused by a lengthy, inflamed appendix wrapping around a loop of the ileum. Aim: to increase clinical awareness of this issue among medical professionals so that it can be identified early and complications prevented. Case presentation: we reported a case of a rare condition, appendicular tie syndrome in a 65 years old diabetic patient. The diagnosis was made intra-operative. Conclusion: Appendicular knot/tie syndrome or appendicular band syndrome all are the names of a very rare presentation of acute appendicitis causing intestinal obstruction. The rarity of this illness makes it extremely difficult to be diagnosed before surgery. In the early stages of inflammation, CT imaging is the gold stander for diagnosis, as well as the high clinical suspicion.References
