A Critical Review of Vatavyadhi and Basti Chikitsa with Special Reference to Musculoskeletal Disorders

Introduction: Musculoskeletal disorders (MSD) include any injury or damage of muscles, nerves, tendons, joints, etc., when under any circumstances these tissues work harder or get stretched beyond their normal limits or when they are used inappropriately (overuse) leading to fatigue of the tissues as well as the body. In Ayurveda the term Musculoskeletal disorders can be correlated to the Nanatmaja Vikaras of Vata-Vatavyadhi (for eg, SandhigataVata, Avabahuk, Gridrasi, etc.) which has specific pathology of manifestation which is comparable to different types of Musculoskeletal disorders). Aim: To understand Musculoskeletal disorders and their management through Panchakarma Chikitsa. Objective: Critical review of literature about Musculoskeletal disorders, Vatavyadhi, Basti through Ayurveda in different Samhita, Modern texts, Pubmed, Index, Google Scholar, etc. Observations: On analyzing the Nidana (Etiology) and Samprapthi (Pathology), we can see clear-cut evidence of the relationship between Pakwasaya (Gut) and the development of Vatavyadhi (Musculoskeletal disorders). This gut microbiology has a close relationship with a wide range of Musculoskeletal disorders and therefore any negative impact, either qualitative or quantitative changes in the composition of the gut microbiota can potentially contribute to the pathophysiology of musculoskeletal disorders. Modulations of gut microbiota could be a possible line of next-generation management of Musculoskeletal disorders where Basti is the most relevant choice. Results: Studies have shown that there is an increase in total serum protein and hemoglobin percentage after Basti administration while ESR tends to decrease, implying that it halts the disease process and improves protein metabolism. Vitamin B12 absorption capacity, intestinal barrier improvement, and microbiome regulation are significantly improved after Basti Chikitsa. Conclusion: Basti serves as the aptest treatment line for Musculoskeletal disorders.
