The Effect of Er,Cr:YSGG Laser Treatment to Healing of Orthodontically Stimulated Gingival Enlargements

Objectives: The objective of this study was to evaluate the healing process of gingival enlargement in adolescents with and without fixed orthodontic appliances by using erbium, chromium:yttrium-scandium-gallium-garnet laser (Er,Cr:YSGG) and scaling-root planning and conventional scalpel for 3 months follow-up. Material and method: 30 patients with gingival enlargement divided in three groups were treated. Control group, without fixed orthodontic appliances, were treated mechanically by scaling-root planning and conventional scalpel only. Test group-1 patients with fixed orthodontic appliances were treated by using Er,Cr:YSGG laser. Test group-2 patients with fixed orthodontic appliances, were treated mechanically by scaling-root planning and scalpel. Patients were called for follow-up in days 1st, 7th, 30th, 60th, and 90th days. Clinical periodontal parameters were compared according to plaque index, gingival index, bleeding on probing, probing pocket depth, and gingival overgrowth index. Inter-group variations in the clinical periodontal parameters were determined at time intervals versus baseline measurements. Results: All groups showed statistically significant reduction in clinical periodontal parameters (plaque index, gingival index, bleeding on probing, probing pocket depth, and gingival overgrowth index) during study period (P<0.05). The fastest and more smooth improvements among groups in healing process were in test group-1 compared to baseline. Conclusion: Er,Cr:YSGG laser therapy is an effective method for patients with gingival enlargement. In this study, laser therapy was applied to adolescents without any anesthesia and appeared no pre-intra-post operative discomfort or pain. Patients with fixed orthodontic appliances have difficulty about self-care, therefore they should be called more often to follow-ups to home-care instructions.
