Intralesional Injection of Triamcinolone Acetonide Effectively Reduces the Post-traumatic and Post-surgical Fibrosis

Intralesional injection of triamcinolone acetonide is commonly used to resolve partially or completely the keloid and hypertrophic scars in the human. This study aimed to show the effect of two successive intralesional injections of triamcinolone acetonide into the fibrous band followed a surgical intervention or trauma in the faciomaxillary area. Its effect was assessed by improving the mouth opening and closing the eye. A total number of 21 (14 males and 7 females) patients recruited from the Consultant Clinics at the Teaching Hospital of Specialized surgeries in Baghdad through 2018, The patients presented with fibrous bands due to surgical intervention or followed traumatic injuries. Two successive intralesional injections of triamcinolone acetonide (40 mg/ml, each injection), 4 weeks apart were done for each patient. The authors measured the mouth opening by vernier caliper and the eye closure by translucent ruler before injection and after 6 months followed-up. Intralesional triamcinolone injection improves the mouth opening from 20.1±5.3 to 33.1±3.7 mm (p <0.001), and restored the eye closure in six out of eight patients. Six-month follow-up did not show any recurrence of the fibrous band. Triamcinolone acetonide reduces the volume of fibrous tissue band, which reflected in the improvement of mouth opening and closing of the eye.
