Metformin is a biguanide commonly used in type 2 diabetes mellitus. About 2% of acute pancreatitis is caused by drugs. Drug induced pancreatitis cases generally do not cause much harm but rarely results in fatality. Metformin is considered to be a safe drug with minimal side effects and only few papers suggest Metformin is associated with drug induced pancreatitis. Among the published case reports the mechanism postulated are drug overdose, drug accumulation and acute renal failure. Management of drug induced pancreatitis requires withdrawal of the offending agent and supportive care.
