The Feasibility of Bilateral Tympanoplasty in Single Sitting in Bilateral Chronic Otitis Media

Abstract

Bilateral tympanoplasty is not a routinely performed surgery by otosurgeons. The reason being is the risk of inducing sensorineural deafness and fear of bilateral ear packing post operatively that may render patient’s total deaf. The aim of the study is to assess the feasibility of bilateral tympanoplasty in single sitting in bilateral chronic otitis media. 50 patients with bilateral inactive mucosal type COM underwent bilateral tympanoplasty in same sitting. The audiological assessment was done using extended fletcher index (EFI). ABG (Air bone gap) was calculated as the mean of the differences between the air and bone conduction thresholds at 500, 1000, 2000 and 3000 Hz. Visual analogue score was used to assess patient comfort and communication difficulty post operatively. The paired “t” test was used to evaluate statistically and compared the outcomes of surgery. Graft take rate was 95% out of 100 ears. There were no bilateral failures. The improvement in hearing thresholds in all groups was highly significant (p< 0.0001) and the improvement in air bone gap in all groups was also highly significant (p< 0.0001). None of the ears had postoperative sensorineural deafness or bone conduction worsening. The procedure is safe and cost effective, reducing work absenteeism, family inconvenience and hospital visits.
